I used to have a pair like the bottom picture, they never slipped off but they did leave a large gap between the sole of my shoe and my foot and felt nice as the air rushed in. The piece of leather that held the main strap to the shoe snapped and so my left heel dragged along the pavement as when I walked in them. The strap around the back of my foot ran from the part covering my toes without anything tethering it to the area beside my heel arch and as such the heel itself did not flip up with the rest of the shoe.
